Kowaliga Veterinary Care offers patient forms online so you can complete them in the convenience of your own home or office.
Download the necessary form(s), print it out and fill in the required information.
Email your completed form(s) or bring them with you to your appointment.
New Patient Registration Form - Required
This lets us record you and your pet's information into our management software so that we can effectively and efficiently provide the service you deserve.
Boarding and Consent Form - Required
This form is required for all boarding reservations and helps us make your pet's stay as comfortable as possible. You may select options such as extra play time or nature walks, list any medications your pet will need, and also list any items from home (toys, bedding, etc.) that you will be bringing.
Surgery Consent Form - Required
By signing this form, you are stating you understand the risks of anesthesia/surgery. We require pre-anesthetic blood work on each surgical patient, to create a personalized anesthetic plan. These precautions allow us to provide the safest anesthesia experience possible for our patients.Microchipping and nail trimming/grinding are other options that can be taken care of while your pet is under anesthesia.

What To Expect
When you are looking for a veterinary care clinic you should be able to count on superior care and excellent service. We have assembled an expert team of veterinary professionals to bring you the best possible healthcare for your pet. We have a state of the art veterinary facility which is clean, comfortable, and efficient. Call (334) 857-1816 to schedule an appointment and we will find a time that is convenient for you.
Once a year, you should take your pet in for a check-up (more often in senior pets). This will include a full physical exam, and we may recommend other treatments as needed. Dr. Mary Battistella or Dr. Josh Morgan will check the health of your pet from head to tail, and you will be very happy you came to Kowaliga Veterinary Care.
Bring any medical records you have for your pet when you come to your first appointment. It's best to keep a journal of your pet's health throughout his or her life, including physical or behavioral changes. If you have something like this, share it with our team. If not, let us know everything that you think will be important. Our doctors can still work with whatever information you have.
In some circumstances, our doctors will refer you to a specialist who has advanced expertise in areas such as internal medicine or surgery.

Payment Options
1. Payment is expected when services are rendered. In order to focus on our patients' needs, customer service and minimizing costs, we do not bill.
2. We accept debit cards, credit cards (Visa, Mastercard, Discover, American Express), checks, and cash. All cards must be signed by the owner of the card. The merchant company automatically adds 3.5% charge to the customers invoice that goes directly to the merchant company.
3. When unexpected illness strikes a pet, unexpected expense strikes as well. Kowaliga Veterinary Care understands this and is able to make some special arrangements through the CareCredit program. It takes just 5 minutes to complete an application and will allow you to break down your payment into 6 monthly installments. You can either apply at our hospital, where we'll call in your application over the phone for immediate approval, or apply yourself online at: www.CareCredit.com.
